Is a thread lift right for a "peanut-shaped" face with hollow, sagging cheeks?

Director Kim Serim ·

With a peanut-shaped face, a thread lift that only pulls can make the hollow areas stand out more. It is often more natural to combine threads (such as Jamber threads) placed in a planned direction to support sagging tissue back into place, with volume treatments to fill the hollow areas.

What makes a peanut-shaped face different?

A face that, from the front, has prominent cheekbones and jawline with sunken side cheeks between them is often called peanut-shaped. With age, cheek fat decreases, and as jaw muscles develop or tissue sags downward, this shape becomes more noticeable.

In a face like this, strongly pulling only the sagging skin upward can leave the hollow side cheeks as they are and make the area below the cheekbones look more sunken. That is why it's important to decide the type and direction of the threads first.

What are Jamber threads?

Jamber threads are cog threads made of dissolvable PDO with spiral barbs. The barbs hold tissue evenly so force doesn't concentrate at one point. They are inserted with a blunt-tipped cannula, which can reduce the risk of damage to blood vessels or nerves.

The threads dissolve over time, and in the process, collagen forms around them, which helps maintain firmness. At Serene Clinic, the expected duration is about 6 months.

Even among cog threads, the shape and thickness of the barbs and how long they take to dissolve differ slightly. We choose based on the facial area and degree of sagging: Jamber threads when even support is needed, MINT or Ultra V when a slightly firmer hold is needed.

Who it suits, and when we plan it with other treatments

Most people who were unhappy with thread lift results had treatment that only pulled, without distinguishing sagging from volume loss. At consultation, we look at the two separately and make a plan.

  • Good fit: people whose facial contour has blurred due to sagging cheeks and jawline
  • Good fit: people who find surgery daunting and want a procedure with short recovery
  • Planned together: if the side cheeks are very hollow, volume is filled with collagen injections or filler
  • Planned together: if the jaw muscles are well developed, Botox is combined

Recovery and precautions

You can return to daily life right after the procedure, but for about 1–2 weeks you may have swelling, a pulling sensation, and discomfort when opening your mouth wide. Rarely, bruising or the feeling of a thread end under the skin may occur, so please contact us right away if something feels wrong.

For about 2 weeks, it's best to avoid facial massage, activities that keep your mouth open for a long time such as dental treatment, and sleeping face down.

Even though collagen formed as the threads dissolve remains, aging continues. After the expected duration, it's best to reassess the sagging and, if needed, plan alternating with other lifting treatments.

Frequently asked questions

Can the threads be seen or felt from the outside?

They are placed in the appropriate layer under the skin, so as a rule they are not visible. Early on, some may be felt, but this usually decreases over time.

How many threads do I need?

It depends on the degree of sagging and face size. We often start from 4 threads for both cheeks, and decide at consultation.

Can I get a thread lift and filler on the same day?

It's often possible. However, we adjust the order and amount considering swelling, and plan it together at consultation.
