How much aegyo filler looks natural, and how long does it last?
Director Kim Serim ·
Aegyo filler adds a small amount of soft hyaluronic acid filler, usually around 0.1–0.3 cc per side, to the narrow band just below the lower lashes to enhance the fullness that appears when you smile. It typically lasts about 6 months to a year, with individual variation. In eyes with bulging under-eye fat it can make the area look puffy, so we check the structure beneath the eye before treatment.

Are the aegyo area and under-eye hollows the same thing?
No, they are different areas. Aegyo-sal is the narrow band, a few millimeters wide, just below the lower lash line that puffs up when the orbicularis muscle contracts as you smile. Under-eye hollows sit lower, along the edge of the orbital bone, where a groove or depression forms.
So aegyo filler and tear trough filler differ in placement, product, and purpose. Aegyo filler is a small design treatment that brings out a smiling look. Tear trough filler fills a hollow groove to soften a tired appearance. In consultation we first sort out which concern you have, and if both are present we plan the order.
Which eyes suit aegyo filler, and which do not?
Aegyo filler suits eyes that look flat below even when smiling, eyes with naturally thin or uneven aegyo-sal, and people whose eyes look sharp and who want a softer impression. The skin below the eye needs some remaining elasticity for a small amount to hold its shape.
If under-eye fat bulges forward, on the other hand, aegyo filler can blend into the fat pad and make the whole area under the eye look puffy. If your eyes often swell in the morning, hyaluronic acid's tendency to draw in water can make swelling more noticeable. When the skin below the eye is very lax, addressing skin elasticity first is usually the better choice.
We postpone treatment if there is inflammation or conjunctivitis around the eye, during pregnancy or breastfeeding, or when an important event is only a few days away. In these cases we do not recommend aegyo filler and explain why.
How is a natural aegyo design decided?
A natural aegyo-sal comes from a thickness that is modest relative to the eye size and a flow that tapers softly from the inner corner outward. At Serene Clinic, our default shape is fullest between the center and the outer part of the eye, thin toward the inner corner, and finished so the outer end does not extend past the outer corner of the eye.
We decide the design by checking both a smiling face and a neutral face. We aim for it to look good when smiling, but if a bulging band remains at rest, it tends to look puffy. The goal is subtle at rest and naturally visible when smiling.
Aegyo-sal is often uneven between sides to begin with. Rather than matching both sides exactly, we look at the eye shape and double eyelid together and set the amount for each side separately to achieve balance.
How many cc of aegyo filler are used, and which products?
Aegyo filler usually uses around 0.1–0.3 cc per side. Even both sides combined usually come to less than 1 cc. It is safer to start with a small amount, review the shape at about 2 weeks once swelling has settled, and top up only where needed.
The skin below the eye is among the thinnest on the face, so a firm filler can feel like a foreign body or look lumpy when smiling. For aegyo filler we choose a soft hyaluronic acid product that spreads evenly, and we adjust the depth so it is not placed too superficially. Filler sitting just under the skin can show through with a bluish tint (the Tyndall effect).
We mainly inject with a thin, blunt-tipped cannula. It can fill the whole band evenly through a few entry points, which helps reduce the chance of hitting a vessel and bruising compared with multiple needle punctures.
Does aegyo filler hurt? How long is the procedure and recovery?
We minimize aegyo filler pain by using numbing cream together with local anesthetic injections. You may still feel a brief sting or pressure as the needle goes in, and sensitivity varies from person to person. Including numbing time, it is usually done within an hour.
Swelling shows easily around the eyes. Aegyo filler swelling and bruising are usually most noticeable around days 2–3 and mostly settle within 1–2 weeks. While swollen, the aegyo-sal looks larger than it really is, so the final shape is judged after about 2 weeks.
- For the first two days, apply a towel-wrapped cold compress briefly and often.
- Avoid spicy and hot foods on the day of treatment.
- Avoid rubbing your eyes hard or pressing on the treated area for 1–2 weeks.
- Light eye makeup is fine from the next day; insert contact lenses carefully without rubbing your eyes.
- Avoid alcohol and heavy sweating exercise for 1–2 days, and saunas and jjimjilbang for about 2 weeks.
- Raising your pillow slightly when sleeping helps reduce morning swelling.
What side effects can aegyo filler have, and when should you contact us right away?
Common reactions are swelling, bruising, soreness at the injection site, and small palpable nodules. Most improve as swelling subsides, but if it still looks lumpy or bluish after 2 weeks, we examine it and adjust. Hyaluronic acid filler can be dissolved with an enzyme injection, so there is a way to reverse it if you are unhappy with the shape.
Rarely, vascular complications can occur when filler compresses or blocks a vessel, and very rare complications affecting vision have been reported with treatments around the eye. If pain gets steadily worse after treatment, the skin turns pale or develops a blotchy net-like pattern, or your vision blurs and your eye hurts, contact the clinic right away without delay.
Because the amount is small, aegyo filler is easy to think of as a minor treatment, but it is an injection near the vessels and nerves around the eye. That is why we check the structure below the eye before treatment and follow your progress afterward.
How long does aegyo filler last, and when should it be redone?
Aegyo filler is expected to last roughly 6 months to a year. The area below the eye does not move as much as the lips, so it tends to last relatively long, but it varies widely depending on the product, the amount, your constitution, and lifestyle.
When you feel volume has decreased, it is easier to keep a consistent shape by checking how much remains and topping up only what is needed, rather than re-injecting the original amount. Repeated top-ups at short intervals can gradually thicken the aegyo-sal so it looks puffy even at rest, so we leave adequate intervals before deciding.

Frequently asked questions
Won't aegyo filler make under-eye fat look more prominent?
In eyes where under-eye fat already bulges, aegyo filler can connect the aegyo-sal and fat pad, making the area look puffier. So we check for under-eye fat protrusion before treatment, and in such cases we either skip aegyo filler or greatly reduce the amount.
Can I get aegyo filler after double eyelid or other eye surgery?
Yes, but it is best done after swelling around the eyes has fully settled. Right after surgery the eye shape keeps changing, so we design the aegyo-sal based on the final eye shape once swelling has resolved. Please let us know the timing and type of surgery in consultation.
Can aegyo filler be dissolved and redone?
Hyaluronic acid filler can be dissolved with an enzyme injection. The eye area swells right after dissolving, so we usually wait about 2 weeks to assess before deciding whether to re-inject.
Can I get aegyo filler and under-eye filler on the same day?
Sometimes both are done on the same day, but filling both areas at once blurs the boundary with swelling and makes it hard to judge the amount. If under-eye hollows are pronounced, we usually suggest treating the tear trough first and refining the aegyo-sal after swelling subsides.
