Why does melasma come back even after laser treatment?

Melasma is not just a problem of pigment in the epidermis. It is a chronic condition involving pigment in the dermis, increased blood vessels, hormones, and UV exposure, so it doesn't go away in one session and often recurs. Results last longer when low-energy toning is split into multiple sessions 2–4 weeks apart, combined with sun protection.

Why melasma recurs so easily

Melasma is a state in which pigment-producing cells overreact. UV, hormonal changes such as pregnancy or birth control pills, and repeated irritation act as triggers. Damage in the dermis and increased blood vessels keep stimulating the pigment cells.

So trying to remove the color all at once with a strong laser can itself become irritation and sometimes makes melasma darker. The principle of melasma treatment is to use low energy over multiple sessions while minimizing irritation.

What role does Clarity toning play?

Clarity is a laser that uses two wavelengths, 755nm and 1064nm. 755nm is well absorbed by melanin and is used for pigment near the epidermis. 1064nm penetrates deeper and is used for pigment in the dermis and the vascular response.

At Serene Clinic, depending on the depth of the melasma and the degree of redness, we choose among Clarity alone, Dual Toning with PicoMajesty, and Triple Toning, which adds Alex toning.

Toning causes almost no wounds on the skin surface, so you can return to daily life right after treatment. However, getting it too often at short intervals can let irritation build up, so we set intervals while watching how your skin responds.

How many sessions, and how far apart?

Toning is usually done at 2–4 week intervals, and many people notice a brighter tone from around 3–5 sessions. However, deep or long-standing melasma may need more sessions.

If the color temporarily darkens or reddens during treatment, we adjust the energy and interval. In rare cases, pigment can be lost, leaving white spots, so we proceed while monitoring progress.

Many faces have freckles, lentigines, and post-inflammatory pigmentation mixed in with melasma. In such cases, toning is applied to the melasma and removal procedures to well-defined spots separately. This lets us even out the overall tone without irritating the melasma.

Daily care to reduce recurrence

It's hard to end melasma with laser alone. Even after treatment ends, continuing sun protection and reducing irritation is the most reliable way to delay recurrence.

  • Apply sunscreen every day, even on cloudy days and indoors, and reapply every 2–3 hours outdoors.
  • Avoid rubbing hard when washing your face or exfoliating often.
  • If needed, we also prescribe topical brightening agents or oral medication.

Frequently asked questions

Will my melasma disappear completely?

Melasma is closer to a condition you manage. It can be made noticeably lighter, but it can come back with UV and hormones, so ongoing care is needed.

Can I get toning in summer?

Yes. But sun protection needs to be even more thorough, and please let us know in advance if you have plans with a lot of sun exposure.

Will moles and spots go away too?

Toning is a treatment suited to widely spread pigment. Well-defined lentigines or moles are better treated with a separate removal method.
