Should neck wrinkles be treated with filler or a thread lift?

If two or three horizontal neck lines are deeply etched, neck filler is the quickest way to fill them. If the whole neck has thinned and lost elasticity, volume thread lifting, which uses dissolvable threads to support collagen production, is a better fit. Vertical neck lines that appear when you lift your head may need Botox, so we first identify the type of wrinkle and then use one treatment or a combination.

Why do neck wrinkles deepen sooner than facial wrinkles?

Neck skin is thinner than facial skin, and there is less fat underneath to support it. It also produces less oil, so it dries out easily, and you bend and tilt your neck countless times a day. The same spot folds again and again, so once elasticity drops, the line stays.

Add to that looking down at a phone or laptop, a high pillow, and sunscreen that stops at the jaw. Natural skin type and build matter too, so many people have clear horizontal lines even in their 20s.

Are horizontal and vertical neck lines treated differently?

Yes. Horizontal lines around the neck come from repeated folding and loss of elasticity, so we approach them by filling the line or firming the skin again.

Vertical neck lines, which stand out at the front of the neck when you clench your teeth or lift your head, come from contraction of the broad neck muscle underneath (the platysma) rather than the skin. Here, Botox to relax the muscle comes before filler or threads.

In reality, the two are often mixed, and how thin the skin has become differs from person to person. So in the exam room, we have you bend, lift, and turn your head to see which lines deepen and when.

When is neck wrinkle filler a good fit?

Neck filler suits people who have two or three deeply etched horizontal neck lines while the skin between them is still fairly firm. Small amounts of hyaluronic acid are placed along each line to fill the groove, so the change is visible from the day of treatment.

Neck skin is thin, so filler injected the way it is in the face can feel lumpy or show through. That is why we choose a product that spreads softly and place small amounts in a shallow layer just under the line. The amount needed depends on the number and depth of the lines and is decided during the consultation.

We minimize pain by using numbing cream together with a local anesthetic injection. Results usually last 6–12 months, and if any areas still look hollow after the first session, we refine them with a touch-up.

Is there an option if I'm hesitant about filler?

For patients who are hesitant about filler, we recommend the Gouri fine-line injection. It combines Gouri with Hilowave and is also used to fill horizontal neck lines naturally.

Gouri is a liquid PCL, so it tends to spread widely after injection. Mixing it with Hilowave moderates this spreading, so it stays where it is injected and fills the creased line locally.

Rather than creating instant volume like hyaluronic acid filler, it gradually fills in the base of the treated area. It suits people who want their neck lines softened naturally without anything looking raised. The number of sessions and amount are decided at your consultation based on wrinkle depth.

How is a volume thread lift different from filler?

Volume thread lifting doesn't fill a single line. It improves the elasticity of the whole neck. Twisted dissolvable threads, such as Jamber or scaffold threads, are placed under the skin. As they are slowly absorbed, collagen forms around them and thin skin becomes thicker and firmer.

Its purpose is different from a typical cog-thread lift that hooks sagging tissue and pulls it up. On the neck, restoring skin thickness and elasticity looks more natural than pulling, so we choose the thread type, number, and direction based on the direction of the wrinkles and the skin condition.

The change builds over 2–3 months rather than showing immediately, and it lasts about 6 months. When there are many fine lines and the skin has thinned overall, it often looks more natural to build the base with a volume thread lift and fill two or three deep horizontal neck lines with filler.

When are injections and threads not enough?

When there is a lot of loose skin under the jaw and fat has sagged along with it, filler or threads alone can't refine the neckline. In that case we recommend Gouri contour lifting together with radiofrequency or ultrasound lifting. Energy-based lifting tightens sagging tissue along the jaw and neck, and Gouri supports collagen production to fill in the hollowed base, giving a smoother neckline. If sagging is severe, surgical options may need to be discussed.

If you are pregnant, take blood-thinning medication, or have inflammation or a skin condition on your neck, the timing and method need to be decided separately, so please tell us during your consultation.

What is recovery like, and what should I avoid afterward?

With both treatments, you can return to daily activities the same day. After neck wrinkle filler, injection marks and mild swelling last a few days, and bruising can last 1–2 weeks, so leave some time before important events. On the day of treatment, avoid spicy and hot foods, alcohol, and saunas.

After a volume thread lift, you may feel tightness or swelling for about 1–2 weeks. During this time, avoid bending or turning your neck forcefully, strenuous exercise, and hot saunas, and use a low pillow. Rarely, you may feel the end of a thread or have swelling and pain on only one side; if so, contact us right away.

What neck care can I do at home?

Daily care cannot erase deep lines that have already formed, but it helps slow the formation of new wrinkles and the fading of treatment results.

  • When applying sunscreen and moisturizer, cover from the jawline down to the collarbones.
  • Raise screens closer to eye level to spend less time looking down.
  • Choose a pillow height that does not bend your neck forward.

Frequently asked questions

Will neck filler make my neck look thicker?

Filler is placed in small amounts in a shallow layer along the lines, so it does not make your neck wider. Too much at once can look lumpy, though, so we split it into sessions when needed.

I'm in my 20s or 30s and have horizontal neck lines. Can I treat them now?

Horizontal lines at a younger age are often strongly influenced by build and posture. Skin elasticity is usually still good, so even a small amount of filler tends to show a clear change. It is best to work on posture as well.

Can I get filler and a volume thread lift on the same day?

Often, yes. We plan to build the base with threads first and then fill remaining lines with filler, and sometimes split them into separate sessions to account for swelling.

Can lasers or radiofrequency improve neck wrinkles?

They help improve skin texture and elasticity overall, but their effect on filling lines that are already deeply folded is limited. That is why they are often used together with filler or threads.
