If neck wrinkle filler left bumps or seemed to wear off quickly, what should change next time?
Director Kim Serim ·
If neck wrinkle filler left the surface lumpy or seemed to fade quickly, the firmness of the product and the degree of correction were likely mismatched to thin neck skin that folds often. Serene Clinic first sorts out whether the problem is mainly deep horizontal lines, thinning skin across the whole neck, or vertical neck bands that appear when you tense (platysmal bands). For the first two, we consider the Gouri fine-line injection, which uses Gouri, a liquid PCL, together with Hilowave; for vertical bands, we consider Botox rather than filler.
At a glance
- What we sort out first
- Mainly deep horizontal neck lines / horizontal lines plus loss of firmness across the neck / vertical neck bands that appear when you tense (platysmal bands)
- Gouri fine-line injection
- A treatment design that uses liquid PCL (Gouri) together with a DHC hyaluronic acid (Hilowave)
- Level of evidence
- Collagen stimulation by PCL is described in the literature; Hilowave data are from cell and animal studies. No comparative clinical trial shows the combination lasts longer
- When changes appear
- Soft correction right after; changes in feel and firmness are observed from about 3–4 weeks (an increase in dermal thickness is not assumed without ultrasound comparison)
- Duration
- Explained as about 5–6 months after one session, over a year after 2–3 sessions (varies by person)
- Vertical neck bands
- Platysma Botox is considered rather than filler. Because of the risk of swallowing difficulty and neck weakness, candidates and injection points are chosen carefully
- Price
- Gouri fine-line injection 199,000 KRW, neck wrinkle filler Belotero 385,000 KRW · Neuvia 550,000 KRW (as of the October 2026 menu)
Why can neck filler look lumpy or seem to wear off quickly?
Bumps after neck wrinkle filler, or the feeling that the lines came back, usually come from a mismatch between neck skin and the properties of the product. Much of the neck has thinner skin than the face, and the same spots fold every time you bend or turn your head.
If a firm, shape-holding filler is placed generously in a shallow layer, the crease may flatten but the surface can stand out. An early report on filler for horizontal neck lines also described temporary irregularity or lines that looked more noticeable[3].
A very soft filler, on the other hand, spreads naturally but may not fully support a deep crease. Swelling right after treatment makes the neck look smooth, and as it settles, the under-corrected parts show again. In this case it is less that the filler was absorbed within a month and more that the initial correction was not enough.
Studies show a similar pattern. In a prospective study of hyaluronic acid filler for horizontal neck lines, improvement was greatest at 1 month, declined somewhat around 3 months, and was maintained through 6 months[4]. In a systematic review of 11 studies, close to half of patients were retreated within a year[5]. This does not mean neck filler does not work, but there are clearly necks where filling alone leaves something to be desired.
What does the Gouri fine-line injection combine, and why was it designed this way?
The Gouri fine-line injection is not Gouri alone. It is a treatment design that adds Hilowave, a hyaluronic acid complex (DHC) product, to Gouri, a liquid PCL (polycaprolactone). It also differs from simply mixing Gouri into a typical HA filler.
PCL is described in the literature as a material that breaks down slowly in the body while prompting collagen formation in surrounding tissue[1]. Gouri is a liquid form of PCL, so there is less concern about palpable nodules than with particle-based collagen injections. Because it is liquid, however, it spreads easily, and keeping it where it is wanted was the challenge.
Hilowave is a complex (DHC) of hyaluronic acids with different molecular weights, and its formulation behaves differently from ordinary non-crosslinked hydrating injections. In models of aged skin, increases in type I and III collagen and elastin were observed[2]. Serene Clinic chose Hilowave for two reasons: it allows horizontal neck lines to be softened without filling them firmly from the start, and it may help Gouri stay near where it was injected. We built that possibility into the treatment design.
Here we separate expectation from established fact. The Hilowave data come from preclinical cell and animal studies, and no comparative clinical trial has confirmed that Gouri actually stays longer, or that the effect lasts longer than with either product alone. Because this involves combining different injectables, the approved scope of each product, compatibility when mixed, and safety are reviewed separately before it is applied on an individual basis.
How do the changes right after treatment differ from those a few weeks later?
With the Gouri fine-line injection, the changes right after treatment differ from those a few weeks later. Right after, the Hilowave formulation is expected to soften horizontal neck lines and give the skin a sense of hydration. It is not a treatment that adds dramatic volume, so some people notice little difference the next day.
The Gouri component is absorbed relatively quickly, and the collagen response is expected to continue while the hyaluronic acid injected with it remains, usually for 2–3 months. In clinic, we look at how the feel, firmness and shape of the lines change from about 3–4 weeks.
That does not mean the dermis will be thicker after 3–4 weeks. Actual changes in thickness can only be known by comparing ultrasound before and after, and the size and speed of the response differ from person to person. Serene Clinic explains duration as about 5–6 months after one session and over a year after 2–3 sessions, but this too is clinical experience that varies by person.
What type of neck lines do you have, and how does treatment differ by type?
Serene Clinic classifies neck lines by what makes them visible rather than how many there are. Even horizontal neck lines of the same depth call for different treatments if the cause differs.
In the second type, the whole neck is treated because Serene Clinic judges that horizontal lines may look less prominent once the firmness of the surrounding skin improves. However, there is no comparative data yet showing a greater effect from treating a wider area, so we present this only as a clinical expectation.
It is common for one person to have all three. In that case we address the most noticeable cause first, one at a time, and decide the next step after seeing the response.
- Mainly deep horizontal lines: two or three deep horizontal neck lines with firm surrounding skin. After checking whether the lines remain when you tilt your head back and whether previous filler is present, only those lines are treated locally.
- Horizontal lines plus loss of firmness across the neck: a neck where the surrounding skin keeps folding even after one line is filled. We consider treating the horizontal lines while spreading Gouri widely across the whole neck.
- Vertical neck bands that appear when you tense: lines that stand out vertically when you pull down the corners of your mouth or clench your teeth. The cause may be contraction of the platysma rather than the skin, so Botox is considered rather than filler.
Why consider Botox rather than filler for vertical neck bands (platysmal bands)?
Vertical neck bands are often not creases in the skin but cords that rise as the platysma, a thin muscle at the front of the neck, contracts, so Botox, which reduces muscle contraction, fits the cause better than filler. A review of platysma anatomy and injection points also treats platysmal bands as a target for botulinum toxin injection[6].
Neck Botox calls for more caution than facial Botox. Muscles for swallowing, speech and holding up the head lie close by, so a high dose or deep injection can cause difficulty swallowing or neck weakness. A case of dysphagia after Botox for neck rejuvenation has also been reported[7].
So platysma Botox is given only after confirming that the bands are actually caused by muscle contraction, in small divided doses into a shallow layer along the bands that need it. If you usually have trouble swallowing, have weak neck muscles, or have a neuromuscular condition, please tell us at your consultation.
If previous neck filler is still there, what do we check first?
If previous neck wrinkle filler can be felt or there are firm areas, we first check what remains before adding any new product. We ask when, which product and roughly how much was injected, and locate it by touch.
If it is hard to tell whether it is leftover filler, swelling or inflammation, we check with ultrasound. If hyaluronic acid filler remains as bumps, we discuss dissolving it with hyaluronidase and plan the next treatment after the skin has settled.
If there is redness, warmth or pain, inflammation or infection is possible, so we treat that first without massage or additional injections. If the lumps are only slightly palpable, waiting and watching without dissolving can sometimes be the better choice.
What are recovery and aftercare like?
With the Gouri fine-line injection, neck wrinkle filler and platysma Botox, you can return to daily activities the same day. Injection marks and mild swelling can last a few days, and bruising, if it occurs, can last 1–2 weeks. Redness, pain and, rarely, infection or palpable nodules can occur, and the degree varies by person.
The PCL component does not dissolve with hyaluronidase, so if a problem occurs, correction differs from HA filler. That is why the Gouri fine-line injection is given in small divided amounts from the start, with follow-up along the way.
Pain is reduced by using numbing cream together with local anesthetic injection. On the day of treatment, avoid spicy and hot foods, alcohol, saunas and exercise that makes you sweat heavily, and do not rub the injected area hard. If you take blood-thinning medication, please let us know in advance.

Frequently asked questions
How many Gouri fine-line injection sessions do I need, and at what interval?
We do not fix the number in advance. The collagen response appears over several months, so we discuss the next session after seeing changes in feel and firmness after the first. Duration tends to be longer after 2–3 sessions, but it varies by person.
I have never had filler. Can I go straight to the Gouri fine-line injection?
Yes. If two or three horizontal neck lines are deep and the surrounding skin is firm, neck wrinkle filler may suit you; if the skin is thin or you are hesitant about filler, the Gouri fine-line injection is considered first. We decide after examining skin thickness and line depth at your consultation.
If my whole neck has sagged, are injections enough?
A neck with loose, sagging skin under the jaw has limits with injections alone. In that case we consider radiofrequency or ultrasound lifting together with Gouri contour lifting.
Can I have this during pregnancy or breastfeeding?
There is not enough safety data on cosmetic injections during this time, so we recommend waiting until after pregnancy and breastfeeding.
